At Connected in Care, our NDIS Mental Health Support Workers provide personalised, one-on-one support to help participants manage their mental health and build daily living skills. This includes assistance with emotional regulation, developing coping strategies, maintaining routines, attending appointments, and engaging in the community. Our support is trauma-informed, culturally inclusive, and designed to promote independence, resilience, and overall well-being.

Our in-home care services provide NDIS participants with personalised support in the comfort and familiarity of their own home. This includes assistance with daily activities such as personal care, meal preparation, medication reminders, light household tasks, and emotional support. We work closely with each individual to promote independence, safety, and well-being, ensuring their needs are met with respect and dignity. Our goal is to help participants feel confident and supported while maintaining control over their daily lives.

We support NDIS participants to connect with others and engage in meaningful community activities. From social outings and group events to volunteering or pursuing hobbies, we help build confidence, reduce isolation, and promote independence — all tailored to your interests and NDIS goals.

Our NDIS Psychosocial Recovery Coaching supports participants with mental health challenges to take control of their recovery. We work alongside you to build a personalised recovery plan, develop coping skills, connect with services, and work toward your goals with confidence and support.
